To develop a coaching alliance that will help me define and reach my goals for an enriched life, I, __________________________ (“the Client”), agree to work with For Growth Coaching, LLC (“the Coach”). Coaching sessions will be conducted via Zoom or another video platform.
As per the International Coaching Federation:
I understand and agree that I am fully responsible for my physical, mental, and emotional well-being during coaching, including all choices and decisions made. I may choose to discontinue coaching at any time.
I understand that coaching is a comprehensive process that may involve various aspects of my life (work, finances, health, relationships, etc.). I acknowledge that I am solely responsible for taking action on any insights or recommendations discussed during coaching sessions, and for the outcomes and results in these areas. I understand that my progress depends on my commitment to applying what I learn, and that the Coach cannot guarantee specific results or success.
I understand coaching does not involve diagnosing or treating mental health conditions. It is not a substitute for counseling, therapy, or professional mental health services. If needed, I will seek appropriate care.
If I am currently in therapy, I have consulted my mental health provider and they are aware of my decision to work with a coach.
I understand certain topics may be shared anonymously and hypothetically with other professionals for training purposes, but my personal details will remain confidential unless required by law.
I understand coaching is not a substitute for advice from legal, medical, financial, or other qualified professionals, and all decisions and actions in those areas are my responsibility.
Confidentiality & Mandatory Reporting
All information shared with the Coach will be kept confidential unless disclosure is required by law. The Coach is a licensed attorney in California and Oregon and is a mandatory reporter. This means that any disclosure of abuse, neglect, or harm to vulnerable individuals must be reported to the appropriate authorities. Please feel free to ask questions regarding this if needed.
Payment Structure
Individual sessions: $499 per hour, payable at the time of booking on the website.
Package of 5 sessions: $2249.
Organizational coaching & workshops: Custom pricing based on content and duration.
Cancellations & Refunds
I agree to provide 24-hour notice for canceling any session. Missed or canceled sessions without proper notice will not be rescheduled or refunded. Both parties may cancel future services with 7 days notice. If the Coach feels I would benefit more from another resource, they may refer me elsewhere and cancel the relationship, providing a refund for unused services.
Non-discrimination
The Coach provides a supportive, inclusive environment and does not discriminate based on race, gender, religion, sexual orientation, age, or disability.
Dispute Resolution & Governing Law
If a dispute arises, both parties agree to resolve it through good faith discussions or mediation. This agreement is governed by the laws of Washington State, and legal disputes will be handled in Snohomish County, WA.
